I think your best bet is to look for a way to sync the Outlook software on your PC with calDAV/cardDAV, as those protocols are going to be supported by Sailfish in the near future

When it comes to the Outlook part your options are unsurprisingly commercial, as the product itself as you can see by e.g. looking at the client list for davical.

The server part seems to be well covered by free and open source software as you can see on this Wikipedia list of server implementations. A popular one for home use seems to be ownCloud.
