What to do with question bait-and-switch?

I just found out a question I voted for got its title and text changed. I find that to be rather bad form, but I do not think the poster was in bad faith.
However, the same technique could be abused to trick the system's users. How can we avoid this?
@mikelima: Please do notify @chemist and/or me if you feel like there's any kind of abuse. We can then initiate discussion and get to clarify intentions. I'm afraid there's no out of the box solution here, it's case by case...
