I have found my phone switched to silent mode a couple of times, and I do not think I ever tried to do that. Has this happened to anybody else?

[update] The mistery is solved, the Pull down menu first item is "Silence", and I did not really noticed that. So either the menu activated when I put the phone in my pocket, or I released the menu too early when trying to activate the camera (which is the only use of that menu, for me , with the default settings).

Still, I feel it is too easy to silence the phone by mistake.

Thank you @Aashish, that's it, that is probably the case. I will update the question with this information, but I stii think it is too easy to trigger.

Are you sure you have not accidentally pulled down the menu on the lock screen and selected turn sounds off by mistake. It can easily happen

Yes this is the problem. But: I find it is easy to trigger the silent mode without noticing.

There is a barred note in the status area, but it is easy to overlook it. For something that may make you miss phone calls, the icon should be much more noticeable. Also, it is too easy to trigger the feature on by mistake.

I thought that the pulldown menu could be customized to remove/move the silence item to the top, so that it was harder to activate by mistake, but the menu customization does not let you change the first item.

I think a better way to enable silent mode would be to make use of the ambiences: Reserve one for the silent mode, and maybe provide one with the phone, with an image that gives a hint that the audio is muted.
